To purchase Special Death Benefit coverage for your employees, you need to complete the enrollment form and return it to INPRS using the address listed on the form. Our finance department will prepare an invoice based on the number of employees to be covered. Payment is due within 30 days of billing. One-year coverage begins on the latter of Jan. 1 or the date payment is received. Coverage is required for the entire staff.
